CMOH participates in most commercial and managed care insurance plans. At your first appointment, we will discuss billing processes with you and go over any concerns that you have. If you have questions about coverage or billing issues, please call us.

If your insurance information changes in any way.
If you are a member of an HMO or managed care plan, you must have a referral from your primary care physician for all visits to this office. We cannot provide services without that referral.
If your plan requires a co-pay, you will be responsible for making this payment at the time of your office visit.
